Why Your Labs Are Normal But You Still Feel Exhausted — And What To Do About It
By Joseph Sinacori, PA-C, MBA | Bespoke Aesthetics, West Palm Beach
It’s one of the most frustrating things I hear from patients.
“My doctor ran every test and said everything looks normal. But I feel exhausted all the time. I can’t lose weight no matter what I do. My energy is gone. My motivation is gone. I just don’t feel like myself anymore.”
If that sounds familiar, I want you to know something important: normal is not the same as optimal. And there is a significant difference between the two.
The Problem With “Normal”
Conventional medicine is designed to identify disease. Lab reference ranges are built around large population averages — they tell your doctor whether you fall outside the range that indicates a diagnosable condition. What they don’t tell you is whether your levels are optimized for how you want to feel, perform, and age.
Testosterone is a perfect example. The “normal” range for men is extraordinarily wide. A man at the very bottom of that range and a man at the top are having completely different experiences of life — in terms of energy, body composition, mental clarity, libido, and motivation. Both are technically “normal.” But only one feels like himself.
The same applies to thyroid function, cortisol, insulin sensitivity, and a range of other markers that conventional labs often overlook entirely or interpret too conservatively.
What I Actually Look For
At Bespoke Aesthetics, when I sit down with a new longevity patient, I am not asking whether your labs are normal. I am asking whether your labs are optimal — for your age, your symptoms, your goals, and your lifestyle.
I look at a comprehensive hormone panel including testosterone, estrogen, DHEA, and thyroid markers. I assess metabolic health, inflammation levels, and nutritional status. I listen to what you are experiencing in your body — because symptoms matter, even when the numbers look fine on paper.
For many patients, what we find is a constellation of markers that are technically within range but far from where they need to be for that person to feel their best. And that is something we can actually address.
What We Can Do About It
Depending on what we find, a personalized longevity and wellness plan at Bespoke might include hormone optimization through testosterone replacement therapy or other hormonal support, peptide therapy to support energy, recovery, and metabolic function, medical weight loss protocols using evidence-based treatments, and comprehensive lifestyle and nutritional guidance to build a foundation that supports everything else.
The goal is not to put you on a protocol and send you home. The goal is to build a long-term strategy that helps you feel like the most energized, capable, and vital version of yourself — for decades to come.
You Deserve More Than “Normal”
If you have been told your labs are fine but you know something is off — trust that instinct. Your body is telling you something. And you deserve a provider who will listen.
At Bespoke Aesthetics in West Palm Beach, that is exactly what Joseph offers. A thorough, personalized evaluation that goes beyond the standard panel — and a real plan to get you feeling like yourself again.
