What is hormone optimization?

Testosterone drives energy, muscle mass, body composition, libido, mood, and cognitive function. Levels decline gradually with age — roughly 1% per year after 30 — and by your 40s and 50s that decline is often enough to notice.

Hormone optimization starts with a different question than most clinics ask: is testosterone actually the problem? When lab work says yes, our approach is to support your body’s own production rather than replace it. We don’t prescribe testosterone at Bespoke Aesthetics. For men whose labs show the signal from the brain has weakened, enclomiphene can prompt your own production to rise — which preserves fertility and avoids shutting down the system you’re trying to repair.

What are the signs of low testosterone?

Common signs of low testosterone include persistent fatigue, difficulty building muscle, increased body fat around the abdomen, low libido, brain fog, mood changes, irritability, poor sleep, and reduced motivation. Many men are told this is just getting older.

It’s worth knowing that these same symptoms are produced by thyroid disease, sleep apnea, iron deficiency, depression, and a number of common medications. That’s why we don’t treat based on symptoms alone. How is this different from testosterone replacement?

Testosterone replacement supplies testosterone from outside the body. It works, but it comes with a tradeoff many men aren’t told about: it suppresses the signal from your brain that tells your body to make its own, and that suppression affects testicular function and fertility.

Our approach starts from the other direction — supporting your body’s own production rather than replacing it. For men who want to avoid indefinite injectable therapy, or who may want children in the future, that distinction is usually the deciding factor.

What that looks like specifically depends on your lab work, and it isn’t the right approach for every man. Some patterns of low testosterone don’t respond to it at all. Joseph Sinacori, PA-C will walk you through what your results show and tell you directly if this isn’t a fit.

Is testosterone replacement right for me?

When a man comes in with fatigue, low drive, poor recovery, or stubborn weight, the first question isn’t which treatment to start — it’s why his levels are where they are. Sleep apnea, thyroid disease, iron deficiency, medications, and metabolic health all move testosterone, and all of them get skipped when a clinic goes straight to a prescription.

Once we understand the picture, our work is on supporting your body’s own production rather than replacing it. Replacement therapy suppresses that production and affects fertility, which is a meaningful decision to make with full information rather than on a first visit.

How long does it take to see results?

Response varies quite a bit between individuals. In general, changes in energy, mood, and libido tend to appear earlier — often within the first one to two months — while shifts in body composition develop more gradually over three to six months. Starting levels, delivery method, sleep, training, and metabolic health all influence the timeline.

Some men respond well. Some respond partially. Some don’t respond, and that’s information too. We monitor with follow-up labs and adjust or reconsider based on what we see rather than on a fixed schedule.

What if I want children in the future?

Testosterone replacement suppresses the body’s own production and can reduce sperm count, which is why fertility is a central part of the conversation before starting it.
